ASSESSMENT & CASE MANAGEMENT:
A comprehensive assessment process is in place to respond to each person’s individual
situation from inception to resolution. Case Managers use a client centered, strength based
approach to direct clients toward self-sufficiency. A case plan is developed and goals and
tasks are set and monitored to move clients toward self-sufficiency.
Crisis House partners with the East County Career Center, Partnerships with Industry, Access and
others to support our job ready client’s employment goals. Transportation support is
provided when to our transitional housing clients.
A mobile medical clinic operated by Family Health Centers of San Diego (FHCSD)
visits our site Monday mornings from 9am to 12 noon.
The clinic provides medical services and dental referrals. This program is made possible through a partnership with San Diego Family Health Centers (SDFHC) for the Homeless Project.
Certified enrollment counselors from SDFHC provide comprehensive health care coverage applications assistance on the 1st & 3rd Mondays 1-4pm and 2nd & 4th Mondays 8:30-12 noon.
UCSD provides Hepatitis C testing and treatment as well.
Case Managers support citizens who reside in East County with applying for utility
bill reduction through SDG&E.
DISABILITIES HELP CENTER
Assists with SSI/SSDI and Veteran Benefits for disabled, people unable to work due to a disability, have been denied benefits or need help filing a disability claim.
Monday & Wednesdays 8:30.-11:30 a.m. No appointment necessary.
LEGAL SERVICES (CIVIC)
