Transitional Housing DISABILITIES
Program Description:

Provides comprehensive long-term transitional housing and intensive case management to homeless males and females with disabilities. Support services include food privileges, counseling, crisis intervention, career exploration, employment development/assistance, General Education Diploma test preparation, transportation, life skill building, conflict resolution, budgeting, alcohol or drug abuse services, referrals to other necessary services and assistance for applying for public assistance. Residential unit(s) house up to 12 individuals, 6 Male, 6 female.

Program Hours:
24-Hours/ 7 Days Residential Units

Eligibilty:
Limited to homeless disabled single adult male and females, to include those with past drug or alcohol addictions, and mild mental health issues. Must be willing to set and work toward long term goals and be able to live in a group environment. Must be sober and drug free. Drug/TB screening a requirement.

Intake:
Referrals & appointment made through community agencies, and homeless shelters. A comprehensive screening and assessment process in necessary. Contact Transitional Housing Coordinator, Disabilities Program at (619)444-1194 ext.303

Transitional Housing Domestic Violence
Program Description:

Crisis House provides comprehensive long-term transitional living and intensive case management to homeless single women with children and single women who have fled a domestic violence situation. Support services include food pantry privilleges, clinical counseling, crisis intervention, group interactions, career exploration, employment, education development/assistance, General Education Diploma test preparation, transportation, life skill building, conflict resolution, budgeting, alcohol and/or drug abuse services, referrals to other necessary services, help with applying for public assistance and 3 months of follow-up case management services.

Program Hours/Days:
24 Hours/7 Days/Transitional living unit.

Eligibility:
Homeless single women and women with children, domestic violence survivors only. Must be willing to set and work toward long term goals and be able to live in a group environment. Must be sober and drug free. Required drug & TB testing. Last DV incidence within last 6 months of application process.

Intake:

Transitional Housing (FOCUS)Families
Program Description:
Crisis House provides comprehensive long-term transitional housing and intensive case management to a homeless family with children. Support services include food pantry privileges, counseling, crisis intervention, career exploration, employment development/assistance, transportation, life skill building, conflict resolution, legal, General Education Diploma test prepration, budgeting, alcohol and/or drug abuse services, referrals to other necessary services, help applying for public assistance, and 3 month follow up case management services. Residential unit houses up to 5 family members.

Program Hours/Days:
24 Hours/7 Days/Residential living unit.

Eligibility:
Limited to homeless families

Intake:
